How to use this assessment

• Answer all 22 questions honestly. The score is only useful if it reflects your real state.

• Score each question on a 0-3 scale: 3 = Yes, fully and documented; 2 = Yes, but documentation incomplete; 1 = Partially / inconsistently; 0 = No / I don't know.

• Your results are calculated immediately and emailed to you.
